Currently, Polish passport applications must be submitted in person at a Consulate. Fingerprints are taken at the time of application, so applications cannot be submitted by mail or through another person, which was allowed many years ago. Consulates accept applications by appointment only, which can be scheduled independently online at www.e-konsulat.gov.pl. If you encounter any difficulties, our office can assist by scheduling appointments free of charge by phone.

Appointment at the Consulate
- A valid identity document (U.S. passport, driver’s license, or state ID)
- A recent passport photograph
- Payment for the passport fee
- Your most recent Polish passport (if available)
- Certified copies of your Polish birth and marriage certificates
In some cases, additional documents may be required, such as a decision confirming Polish citizenship or a decision from a Polish Civil Registry Office regarding a change of your first or last name.
Supporting documentation
Passport eligibility is determined by the Consulate based on the application and supporting documentation. Applications are now generated electronically from official Polish registers of vital records, so it is essential that the information in these registers matches your identity documents, including first and last name, middle name, and marital status. If you have a middle name, bring an ID showing the full spelling rather than just the initial. Any discrepancies must first be corrected in Poland, or on your U.S. ID. For example, a last name change after marriage must be registered with the Office of Civil Registration (see foreign marriage registration), changes to first names after naturalization (e.g., Grażyna → Grace) require an administrative name change, your marital status must also match Polish records, so a divorce or spouse’s death must be officially registered in Poland.
In some situations, confirmation of Polish citizenship may be required before applying. This typically applies to those whose passports expired long ago, individuals born abroad who have never held a Polish passport, or cases where document details do not match birth or marriage records.
